For legal persons
Application form:
1. A copy of certificate confirming state registration of legal person;
2. A copy of relevant document on applicant’s registration by tax authorities;
3. A copy of document confirming applicant’s right to use (right to ownership, lease, use, etc.) per facility specified in the application;
4. Copies of documents confirming applicant’s right to use (right to ownership, lease, use, etc.) technological, experimental measuring devices, software tools and other equipment necessary for carrying out this type of activity;
5. Copies of documents confirming availability of the applicant’s experts with appropriate education, qualification and at least 3 years of work experience in this type of activity including in information technologies sphere;
6. Copies of certificates of conformity on equipment, appropriate software and technical support facilities, as well as means of information protection to be used for this kind of activity by the applicant.
7. Conclusions of the Ministry of Internal Affairs of Azerbaijan Republic, Ministry of National Security of Azerbaijan Republic and Ministry of Health of Azerbaijan Republic for carrying out this type of activity by the applicant;
8. A copy of the document confirming protection of applicant’s activity scope;
9. Document confirming payment of state due for obtaining license, in the absence of refusal to issue a license.
