2. A copy of certificate on state registration of the legal entity (for physical persons: name, patronymic, surname, ID card details (serial number, when and by whom issued, address), personal identification number (after putting PIN into service), address];

3.A copy of certificate on the applicant’s registration by tax authorities;

4. A copy of document confirming applicant’s right to use (right to ownership, lease, use, etc.) at the address of activity of the applicant;

5. A copy of special permission (license) in case of request on allotment of numbering resources for organization of service on type of activities requiring special permission (license) in telecommunications sphere;

6. List of telecommunications services provided for use of numbering resources and structure chart of telecommunications network to be created;

* The application should provide detailed information on type of activity of legal or physical persons, and usage purpose of the requested number resources.
