1. Application for reception of special permission: For physical persons (.doc)   ,   For legal persons (.doc)   

For legal persons: name, legal address and kind of foreign economic operation carried out by obtaining special permission;

for physical persons: name, patronymic, surname, information on ID card (serial number, date, name and address of the body issuing the document), kind of foreign economic operation carried out by obtaining special permission;

2. Copy of certificate confirming registration of the customers (applicants) by tax authorities;

3. Information on receiver (sender) and end user;

4. Information on working life of the product and technical specifications;

5. Copy of signed agreement (contract);

6. Original certificate of origin for exported goods;

7. Additional documents, required depending on the type of goods.
