on legal entities : name, legal address, type of business activity;

on physical persons: name, patronymic, surname, information on ID card (serial number, when and by whom issued, address), type of business activity;

2. Copy of relevant document confirming applicant’s state registration by tax authorities;

3. Copy of license in cases provided in the legislation;

4. Conclusion of relevant executive body in cases provided in the legislation;

5. Relevant decision of State Commission on Radio Frequencies;

6. A copy of certificate issued for the equipment in accordance with the existing legislation;

7. Document on organization of protection the relevant facilities, provided for storage.
